It is important that players planning to claim, carefully read the FNSW Insurance document prior to making a claim.

If you have a season ending injury DO NOT de-register if you intend to make an insurance claim.

Once you have completed the online Insurance claim the Club Declaration needs to be signed by BHFC President or Secretary ONLY. Coaches or managers or any other witnesses are not an appropriate authority to sign this declaration.

All completed insurance claims should be submitted online within 60 days of the date of the injury. DO NOT wait for your entire injury treatment and documentation to be completed before you lodge your claim.

Your insurance claim is between yourself and FNSW Insurer, not BHFC. All correspondence and questions should be directed to the governing body’s insurance company.
